Registration Form Instructions
- Please pre-register first if you haven't already done so.
- Download the electronically fillable Registration and Polo Reservation Form (in pdf format).
- Fill the form out. You can use the Reset button at the end of the form to clear the fields.
- Use the Print button to print the filled form.
- If you are paying with a credit card, fax the first page of the form to +1 336 758 6142. If you are paying by check (US participants only) make the check payable to "Wake Forest University" and mail the form with your check to:

- A receipt confirming your payment will be e-mailed to you within approximately 3 days. A signed original receipt will be given in your packet upon your arrival at SCINT. If you need a signed original receipt by postal mail earlier than the conference, please indicate that by e-mail response when you receive the first e-mail confirmation of payment.
On-site payments can be made by credit card, check or cash.

* SCINT participants who register at the non-member rate may apply at the conference for a half-year of IEEE and NPSS membership at no additional charge. The half-year membership covers the period July 1-December 31, 2007.
** Proof of student status is required. Registration at the Student rate entitles the participant to all conference activities and events except the proceedings and the banquet which can be purchased at registration separately. It is hoped that providing students with proceedings and the banquet or an alternative may become possible through sponsor support.
